I tried searching this up online but I can't find a good solid definition. I just need it summed up. Anyone happen to know what it is or have a good example?

It's more usual to speak of a "State Transition" diagram, a representation of an entire system by a sequence of states which describe its behavior. This is a fair example of the process:

What is the context? Are you refering to state transitions in automata (like a finite state machine)? Or something else?
